Quitting cold turkey if you are pregnant or nursing is always the best choice. The standard recommendation is to not use NRT. The largest trial of nicotine replacement in pregnancy showed that although women quit at higher rates at first, they relapsed and began smoking again at rates similar to women who didn’t use NRT.
